Our Diabetes Self-Management Education Program
Learning that you have diabetes can be scary. Chances are you are experiencing a range of emotions and have many questions racing through your mind. Our Diabetes Management Team is here to calm your fears by answering all of your questions and provide the support you need. While there is no cure for diabetes, learning selfmanagement skills can help delay or prevent long-term complications. Our Diabetes Self- Management Education Program offers you a comprehensive approach to diabetes selfmanagement education. Our diabetes educators will create a customized program to meet your unique needs that will empower you to make good choices to help keep you healthy. The information and support we provide will give you the tools to manage your diabetes with confidence.
Our Team's Approach to Your Care
Learning to regulate your blood sugar levels through a blend of good nutrition, exercise and, often, medication, is crucial to successfully controlling your diabetes. Our tailored outpatient self-management program begins with an initial personal assessment.
Then, our caring staff will work closely with your referring physician while educating you about diabetes. You will learn about home glucose monitoring, healthy nutrition, benefits of exercise, and healthy coping, so that you can control your diabetes and take pleasure in a better quality of life.
Within one to three months after your first appointment, we will follow up with you to monitor your progress and/or ongoing needs.
Our nationally-certified diabetes educators are guided by a board-certified endocrinologist, and include registered nurses and registered dietitians.

- Sweet Success program affiliate of CDAPP for diabetes care and management for pre-pregnancy and during pregnancy

- Free professionally facilitated monthly support groups
Referrals and Insurance
Participation in the outpatient Diabetes Self-Management Education Program requires a referral from your physician/ healthcare provider. Medicare, Medicaid and most third-party insurances are accepted.
Nationally Recognized Care
The Diabetes Self-Management Education Program at Providence Little Company of Mary Medical Center Torrance was the first in the South Bay to be recognized by the American Diabetes Association as a quality diabetes self-management education program. This award is based on a rigorous clinical review process every three years to ensure high standards of diabetes care and education.
