When your doctor feels that you are ready to leave the hospital he or she will authorize a hospital discharge. Please speak with your nurse about our discharge procedures. Here are few tips to make the discharge process run smoothly:
- Be sure you and/or your caregiver have spoken with a discharge planner and that you understand what services you may need after leaving the hospital.
- Verify your discharge date and time with your nurse or doctor.
- Have someone available to pick you up.
- Check your room, bathroom and bedside table carefully for any personal items.
- Make sure you or your caregiver has all necessary paperwork for billing, referrals, prescriptions, etc.
Discharge time is 11 a.m. When you are discharged, a member of the hospital staff will escort you to the front entrance and help you into the car. After your hospital stay, please bring your hospital discharge instructions with you on your next visit to the doctor. You should make arrangements with a family member or friend to drive and help you home.
Information You Need Before You Leave the Hospital
This is an overview of why you were in the hospital, which health care professionals saw you, what procedures were done, and what medications were prescribed.
This is a listing of what medications you are taking, why, in what dosage, and who prescribed them. But also having a list prepared by the hospital is a good way to doublecheck the information.
As a convenience to our patients, we provide free delivery to patient' rooms for discharge prescriptions via the Providence Pharmacy, located on the first floor of the Disney Family Cancer Center. Free shipping via email service is also available upon request. If you prefer to pick up prescriptions at the Providence Pharmacy, you are welcome to enjoy our serene Meditation Garden or visit our Resource Library at the Cancer Center. The Providence Pharmacy is located one block North of Providence Saint Joseph Medical Center at 181 S. Buena Vista Street, and accepts all major prescription insurance plans. All major credit cards are accepted. Simply call 1-818-748-4950 or ask your physician to send your prescription to the Providence Pharmacy.
Follow-Up Care Instructions
Make sure you have paperwork that tells you:
- what, if any, dietary restrictions you need to follow and for how long.
- what kinds of activities you can and can'Äôt do, and for how long.
- how to properly care for any injury or incisions you may have.
- what follow-up tests you may need and when you need to schedule them.
- what medicines you must take, why, and for how long.
- when you need to see your physician.
- any other home-care instructions for your caregiver, such as how to get you in and out of bed, how to use and monitor any equipment, and what signs and symptoms to watch out for.
- telephone numbers to call if you or your caregiver has any questions pertaining to your after-hospital care.
Outpatient Transportation Service
Providence offers transportation service to patients (who are unable to drive) living within a 10-mile radius of the Medical Center. The departments you are visiting (Maternity Center, Cardiac Rehab, Radiation-Oncology, Day Surgery and Physical, Occupational, or Speech therapy) will be happy to schedule the Care Van for you.
