Providence Southern California provides a range of long-term care for patients who do not need treatment in an acute-care hospital, but still require daily nursing care. A patient’s physician will determine the necessary level of care.
Whether it’s transitional care from hospital to home, a skilled nursing facility or sub-acute care, Providence offers quality and compassionate care for patients in need of 24-hour expertise.
Providence Little Company of Mary Medical Center San Pedro and Providence Holy Cross Medical Center in Mission Hills provide sub-acute care for long-term patients. Some patients come to rehabilitate and return home. Other patients make our Sub Acute Care Centers their home.
These Sub Acute Care specialize in the care of ventilator and tracheostomy dependent patients. Sub-acute patients are medically fragile. They require special services, such as inhalation therapy, tracheotomy care, intravenous tube feeding, and complex wound management care.
Medical conditions that may require ventilator care include respiratory failure, spinal cord injury, cerebral vascular accident, severe heart attack, traumatic brain injury and multiple sclerosis. Our skilled medical teams of physicians, nurses, therapists, dietitians and case managers work together to care for our sub-acute patients. Our goal is to create an environment that will minimize the progression of the disease, aide in preserving the patient’s stability and return the individual to the highest level of function or independence as possible.
Patients admitted to sub-acute care must meet the Department of Health Services criteria.
