Is your doctor moving and becoming part of Santa Monica Family Physicians? These are some of the most frequently asked questions and answers that should help alleviate any concerns you might have. If one of your questions is not on this list, please call 310-829-8441 between the hours of 8 a.m. and 5 p.m., and one of our service representatives can help.   

Why is my doctor leaving UCLA to form a new medical group?

The doctors have chosen Saint John’s and Providence because they feel that this affiliation will allow them to simplify your access to quality health care.

Why did Santa Monica Family Physicians choose to affiliate with Saint John’s?

Some of the physicians in this group have had ties to Providence Saint John’s and to our community for a very long time. We like the community-based physician culture found at Saint John’s and the ease of navigating the campus. Our patients will have access to a number of world-renowned specialists for orthopedic, heart, cancer, neuroscience and other specialty care. Additionally we have access to the John Wayne Cancer Institute where physician researchers are dedicated to finding a cure for cancer. Clinical trials are available through this institute.  

Will my doctor stay at current address?

No, they are not staying at the current address, but will not be moving too far away!

Will SMFP take my insurance?

SMFP takes Aetna, Anthem BC, Blue Shield, Cigna, Choice Care (Humana), Coventry, Galaxy, Health Net, Interplant/Health Smart, Medicare, Multiplan, Motion Picture, Oscar, PHN (Providence Health Network), Medicare and United Healthcare. The group is actively negotiating more plans and will continue to update this list on our website as new plans are added. Please visit our website on a regular basis for the most current updates.

Can I continue to use my doctor as my primary care physician after he/she departs the current practice?

Our goal is that you can continue to keep your long-standing relationship with your doctor. Please contact us should you have any questions or need help navigating the insurance process. We will be actively updating our website providing new information that may be useful to you, so we invite you to visit this page and look for updates.

How do I transfer my medical records?

Your physician will utilize Epic, an electronic health records program that is used throughout Providence and at Cedars, UCLA and Kaiser. This allows your care team to pick up right where you left off, with your most current medical record. If you're not with one of these providers, we'll have to establish a new medical record for you in our practice. The good news is with EPIC, you will also be able to view your lab and test results through an online portal called MyChart.  

Can patients still see their specialists at UCLA?

We want you to continue relationships with the specialists you’re most comfortable with. If you do want to make a change, Providence Saint John’s Health Center has nationally renowned specialists in a number of areas, including but not limited to cancer, orthopedics, neurology and cardiology.

Can I make an appointment now, if I need a follow-up with my doctor after July 1?

Yes. If you call 310-829-8441, schedulers can book appointments now for any visits occurring after July 1, 2017. Soon you will be able to book through online scheduling via our website. Any visits prior to July 1 should be made the same way you are currently scheduling.   

Will you still offer extended hours and urgent care?

Extended care will be offered later in the year. Keep visiting our website for updated information.
