Do you accept my insurance?
A: We participate with most major health plans and many managed care plans, including Medicare Advantage. For the most complete information about participating providers, please contact your health plan.

Can I receive care if I don’t have insurance?
A: It is our Mission to provide care to our communities and we provide you with options. You are required to pay at the time of service and we do accept cash, check or credit card. A discount for payment in full at the time of service is an option. Please discuss this with our patient representative at the time you schedule an appointment.

For adults in the South Bay area, you have the option to receive care from our Vasek Polak Health Clinic in Hawthorne for a flat fee of $50.

Why do you require a copayment at the time of service?
A: Copayments are required by your health plan.

How can I pay my bill?

  • Mail – send your payment by mail once you receive your bill
  • In Person – you can leave your payment with anyone of our medical offices
